Your passport photo must be taken within the last 6 months, and it needs to be exactly 2×2 inches with your head measuring between 1 and 1⅜ inches from chin to crown. You must have a neutral expression, both eyes open, mouth closed, facing directly forward. Miss any of these specifications and your application gets rejected.
